This form is for students returning to Eden Dance Academy. By completing this form you still agree to the terms & conditions of enrolment. Your child's publishing status will remain the same as previous enrolments until Eden Dance Academy is notified. 

Eden Dance Academy will select classes based on the previous years level. If you have any notes for us, there is a section at the end of this form.
